Assessment Clinician, OSP

Hyacinth is a Registered Social Worker with the Ontario College of Social Workers and Social Service Workers (OCSWSSW). Hyacinth is a graduate of York University having achieved her Master of Social Work Advanced Standing degree in November 2022 and Bachelor of Social Work Honours (2015).
Hyacinth is an advocate for mental health and recovery-oriented approaches informed by her over 10 years of work experience. Hyacinth is passionate about being a welcoming connection for those seeking to enhance their personal lives through therapy and believes that when assisting clients’ best practices such as active listening, genuine interest, and engagement can enhance their experience with the OSP Program.
Before joining the Care Point Health team, Hyacinth worked for CMHA Peel Dufferin as an Outreach Worker providing street-based and community-based outreach services to unhoused individuals that included housing securement, connection to community services, clinical assessments, case management, and mental health and addiction recovery supports.
Hyacinth also worked for Services and Housing in the Province (SHIP) as a Crisis Support Worker supporting adults often unhoused with complex mental health and substance needs, at risk or experiencing a crisis, at risk or has come into involvement with the criminal justice system completing intakes, crisis and short-term case management discharge plan, coordination, and referral to other community support services.
